1. The Science of Fibrin Amyloid Resistance
In healthy blood, clot formation is transient and dissolved by the endogenous enzyme plasmin. However, pioneering research by Professor Resia Pretorius and Professor Douglas Kell (PMID: 34425843) revealed that in Long COVID and ME/CFS, inflammatory cytokines and viral proteins cause fibrinogen to polymerize into anomalous beta-sheet-rich amyloid microclots.
These anomalous clots are structurally resistant to physiological enzymatic fibrinolysis. They become trapped in terminal capillaries (which are only 5 to 7 microns in diameter), obstructing erythrocyte transit and starving tissues of oxygen.
2. Comparison of Fibrinolytic Agents
- Lumbrokinase (Earthworm Protease): The clinical gold standard for microclots. Composed of six isoenzymes that exhibit dual mechanisms: they act directly as fibrinolytics and act indirectly as plasminogen activators. Importantly, lumbrokinase only dissolves fibrin when fibrin is present, dramatically reducing bleeding risks.
- Nattokinase (Fermented Natto Soy Extract): Derived from Bacillus subtilis. In addition to cleaving cross-linked fibrin, nattokinase cleaves and degrades PAI-1, which unblocks the body's natural tissue plasminogen activator (tPA).
- Serrapeptase (Silkworm Enzyme): Highly effective at breaking down cellular biofilm, non-living proteinaceous exudates, and inflammatory kinins, making it a powerful synergistic partner for nattokinase.
3. Administration Protocol & Die-Off Mitigation
To ensure safety and efficacy, clinicians advise the following clinical guidelines:
- Strict Empty Stomach Timing: Must be ingested with water at least 45 minutes before eating or 2 hours after a meal.
- Start Low and Titrate: Rapid microclot breakdown releases entrapped pro-inflammatory cytokines into the systemic circulation, causing a temporary flare in malaise (the "microclot Herxheimer" reaction). Start with 2,000 FU nattokinase daily for 1 week before escalating.
- Surgical Precaution: Discontinue all systemic enzymes at least 10 to 14 days prior to any planned dental extraction or surgical procedure.
